【Medical and Dental】Tuition Fee Exemption / Deferment, Admission Fee Exemption / Deferment — Application Forms Download (1st Semester of the 2026 Academic Year)

Applicants (medical and dental fields) who wish to apply for tuition fee exemption/deferment or admission fee exemption/deferment for the first half of the 2026 academic year should download the forms below and complete their application within the application period.
Be sure to check the following: “【Important】 About the application procedure” and “【Must read】 Instructions for filling out”.

If sufficient supporting documents are not submitted or if there are deficiencies in the documents, exemption or deferment cannot be granted.
If it is found that the contents of the submitted documents differ from facts, any granted exemption or deferment will be revoked.

【Notes】

Application document preparation and submission

  • For STEP 1, convert all application documents to PDF files before submission. (Word, Excel, photos/images (screenshots, etc.), etc.)
  • For items in the application documents that require a handwritten signature (name, address, etc.), print the document, sign by hand, then scan and submit as a PDF file. (For items that do not require a handwritten signature, it is acceptable to fill in or create them on a computer.)
  • When submitting, combine PDF files into one file per person (ZIP file).
  • For STEP 2, some documents require submission of originals.
    • For documents that require original submission, keep them safely until the STEP 2 application period. Photos (images taken with a smartphone, etc.) or copies are not acceptable.
    • If you cannot submit an original among the documents that require originals, be sure to consult in advance. (Replacements or returns after submission are not possible.)
  • Documents that include Individual Number ("My Number(マイナンバー)") will not be accepted.
    Obtain a copy of the certificate of residence that does not include your My Number.
  • Do not alter submitted documents with blacking out, etc.
  • When filling out, use black or blue ballpoint pen. (Do not use erasable pens.)
  • Carefully confirm that there are no errors such as incorrect fiscal years in the documents before submitting. If the fiscal year is incorrect, the application will be denied, so check carefully before submission.

About “Certificate of Residence(住民票)”

  • Confirm that the lower part of the certificate of residence contains the statement: “This copy is certified to have no difference from the original resident record of all household members.(この写しは、世帯全員の住民票の原本と相違ないことを証明する)” (If this statement is missing, the application will be denied.)
  • Originals are required for STEP 2. Keep them until the STEP 2 application period. Photos or copies are not acceptable.

About “Taxation Certificate (Non-taxation Certificate)(課税証明書(非課税証明書))”

  • Submit the “Reiwa 8 fiscal year(令和8年度) Taxation Certificate (Non-taxation Certificate)” (i.e., the 2026 fiscal year certificate proving income in calendar year 2025).
  • The “Reiwa 8 fiscal year Taxation Certificate (Non-taxation Certificate)” will be issuable from around late May 2026.
    (Depending on the issuing municipal office, issuance may begin around early June 2026.Please check the issuance availability with the issuing municipal office. Obtain it by the STEP 2 application period.)
  • If the fiscal year is incorrect, the application will be denied, so confirm carefully when obtaining.
  • Taxation certificates (non-taxation certificates) are issued by the municipality where you resided on the tax assessment date (January 1).
    If you moved after the assessment date, request issuance from the municipality where you resided on January 1, 2026.
  • 【Mainly for newly admitted international self-funded students】
    If you arrived in Japan on or after January 2, 2026 and cannot obtain a taxation certificate (non-taxation certificate), submission is not required.

Application forms (download from below)

“Japan Nationality, etc.” “Self funded International Students” “Undergraduate Students” and “Graduate Students” require different mandatory submission documents, so please be aware of this.

“Japan nationality, etc.”: In addition to persons holding Japanese nationality, this category applies to foreign nationals whose residence status falls under any of the following.

Persons who fall under “Japan nationality, etc.”
  • Special Permanent Resident as defined by law(法定特別永住者)
    • “Special Permanent Resident” refers to those defined in the Act on Special Provisions Concerning Immigration Control for those who left Japanese nationality based on the Treaty of Peace with Japan (Act No. 71 of 1991)
  • Persons whose residence status is “Permanent Resident(永住者),” “Spouse or Child of Japanese National(日本人の配偶者等),” or “Spouse or Child of Permanent Resident(永住者の配偶者等)”
    • Residence statuses are determined pursuant to the Immigration Control and Refugee Recognition Act (Act No. 319 of 1951).
  • Persons who are “Long term Resident(定住者)” and have the intention to obtain permanent residency in the future
  • Persons with “Dependent(家族滞在)” residence status who meet all of the following conditions 1–4
    • Persons who first entered Japan before graduating from elementary school in Japan, or who graduated from an elementary school in Japan (including the lower school division of compulsory education schools and the elementary division of special needs schools)
    • Persons who graduated from a Japanese junior high school (including the upper school division of compulsory education schools, the lower division of secondary education schools, and the junior high division of special needs schools)
    • Persons who graduated from a Japanese senior high school, etc. (including the upper division of secondary education schools and the upper division of special needs schools; also including those who have completed the third year of a college of technology, those who completed a specialized training college’s upper course (limited to courses with a curriculum of three years or longer), and those who passed (or are expected to pass) the Examination for the Certificate of Graduation from Upper Secondary School)
    • Persons who, after graduating from graduate school, intend to work in Japan and settle here

  • Responses by the academic supervisor (in principle the division head) via Forms.
    Inform the supervisor of the Forms link and ask them to respond within the application period.
  • There is an item where the applicant’s student number must be entered. Provide your student number to your supervisor when requesting the Forms response.
  • If there is no response within the period, the application may be denied, so please be careful.
